Evidence-Based Invasive Treatments for Cerebral Vasospasm Following Aneurysmal Subarachnoid Hemorrhage

چکیده
Starting in 1984, the technique of angioplasty was no longer confined to only the treatment of peripheral and coronary arteries. In the original paper investigating the technique of angioplasty in cerebral arteries after cerebral vasospasm from aneurysmal subarachnoid hemorrhage (aSAH) , Zubkov et al. found an overall decrease in headaches and focal neurological deficits after the procedure (Zubkov et al, 1984). Despite advances in both medical and endovascular treatment of cerebral vasospasm since then, vasospasm remains a prominent source of morbidity and mortality for patients in the Neuro-Intensive Care Unit. At an estimated incidence rate of 10-28/100000 people, aSAH is associated with a 20% to 40% risk of development of symptomatic, cerebral vasospasm. Of these patients experiencing symptomatic vasospasm, an estimated 10-15% will die before medical therapy while the other 85-90% will have an overall mortality rate of 32-67% (Weant et al, 2010; Frontera et al, 2009). In a review of the literature investigating endovascular treatment of cerebral vasospasm, an absence of standardization is present across these studies. Starting from the basics, the literature has not produced a “gold standard” definition of vasospasm. While some groups define vasospasm by a clinical ,neurological deterioration (Andaluz et al, 2002), others use a variety of diagnostic modalities, such as Transcranial Doppler Velocities (Oskouian et al, 2002), digital subtraction angiography (Frontera et al, 2009), and narrowing of vessel diameter via CT angiography (Coenen et al, 1998), to make the same diagnosis. Therefore, assessing the overall efficacy of angioplasty for cerebral vasospasm is difficult when the literature provides different indications for the same treatment. Once a diagnosis of cerebral vasospasm is confirmed, a lack of standardization continues throughout its treatment. While some groups have associated a good neurological grade

ANATOMIC CORRELATION BE TWEEN INTIMAL PATHOLOGY AND CEREBRAL VASOSPASM FOLLOWING SUBARACHNOID HEMORRHAGE
Subarachnoid hemorrhage (SAH) resulting from a ruptured intracranial aneurysm can induce cerebral vasospasm with subsequent reduction in cerebral blood flow (CBF). The present study examines the pathological alterations in the wall of human cerebral arteries at autopsy, especially with regard to intimal pathology, following aneurysmal SAH. Arterial segments from the circle of Willis were fi...

Cerebral vasospasm is a major source of morbidity and mortality in patients with aneurysmal subarachnoid hemorrhage (aSAH). Evidence suggests a multifactorial etiology and this concept remains supported by the assortment of therapeutic modalities under investigation. The authors provide an updated review of the literature for previous and recent clinical trials evaluating medical treatments in ...
متن کاملCORRELATION BE TWEEN ENDOTHELIAL INJURY AND CEREBRAL VASOSPASM FOLLOWING A DOUBLE SUBARACHNOID HEMORRHAGE IN THE RAT
While a wide array of pathological changes occur in cerebral arteries following subarachnoid hemorrhage (SAH), the most consistent is endothelial damage. Since the endothelium normally modulates reflexes that influence vascular tone, any damage to it may represent a significant contributor to cerebral vasospasm following SAH. This experimental study investigates the correlation between end...

Current clinical practice standards are addressed for the invasive interventional management of post-hemorrhagic cerebral vasospasm (PHCV) in patients with aneurysmal subarachnoid hemorrhage. The conclusions, based on an assessment by the Standards Committee of the Society of Neurointerventional Surgery, included a critical review of the literature using guidelines for evidence based medicine p...

Cerebral vasospasm considered to be a serious cause of morbidity and mortality following subarachnoid haemorrhage (SAH).Despite several available therapeutic options, current protocols do not prevent major consequences of vasospasm. Inflammation is believed to play an important role in post-haemorrhagic vasospasm. Meloxicam is a non-steroidal anti-inflammatory drug. The aim of this study was to...
متن کاملCurrent Controversies in the Prediction, Diagnosis, and Management of Cerebral Vasospasm: Where Do We Stand?
Aneurysmal subarachnoid hemorrhage occurs in approximately 30,000 persons in the United States each year. Around 30 percent of patients with aneurysmal subarachnoid hemorrhage suffer from cerebral ischemia and infarction due to cerebral vasospasm, a leading cause of treatable death and disability following aneurysmal subarachnoid hemorrhage. Methods used to predict, diagnose, and manage vasospa...
